information that users, visitors, and customers (“you” or “your”) provide to us when using the Site, and (2) certain information about you that is automatically collected when you access or use the Site. This Policy also describes
certain rights and options that you may have with regard to your personal information. This Policy does not apply to (1) information collected by us offline, or (2) information collected through any third party (including our affiliates
and subsidiaries) and any third party application, website, or content (including advertising) that may link to or be accessible from or on the Site. Please read this policy carefully to understand how we treat your personal

condition, if applicable, as well as demographic and other information relevant to diagnosis and treatment such as your condition, lifestyle and general medical history (e.g. allergies, other medications). This information is necessary
to enable us to provide you the account you have requested and to maintain the account and your profile. You may update your account information by editing the information associated with your account.
Location Information
When you use the Site, we may collect information about your use of the Site, including your interaction with advertising and analytics services on the Site, in order to (a) serve you the content and functionality you request, and (b)
maintain the privacy and security of the Site and services. Location information collected may include your Internet Protocol (IP) address or unique device identifier.
Cookies and Similar Technologies

Automated Information

Email Interconnectivity
If you receive email communications from us, we may use certain tools to capture data related to if/when you open our message and if/when you click on any links or banners it contains. Other information collected through this email

information to enhance our marketing efforts. We do not sell or distribute this information to third parties
Feedback / Support / Inquiries
If you provide us with feedback or contact us for support or to ask us questions, we will collect your name, address, telephone number, email address, and other information needed to respond to your feedback, or request for support, or
to answer your question.
Mailing List
When you sign up for one of our mailing lists, we collect your contact information, including your email address. We will not include you on any mailing list unless you sign up for the mailing list(s). You may request removal from our
mailing list(s) and we will remove you from the mailing list(s) within five days of our receipt of your request.
Your Communications
When you communicate with us, we may retain your communications in order to process and respond to them, as well as to improve our services. We may use your email address to communicate with you about our services or send you periodic
emails. If at any time you would like to unsubscribe from receiving future emails, you may unsubscribe at the bottom of each email.
Financial and Payment Information
If you choose to purchase goods or services from us, we may collect your name, address, telephone number, bank account number, bank routing information, and other data necessary to process payments, including credit card numbers,
security codes, expiration dates, and other related billing information. By submitting your payment card information, you expressly consent to the sharing of your information with third-party payment processors and other third-party
services (including but not limited to vendors who provide fraud detection services to us and other third parties).
Third Party Information
In addition to the information that we collect from you directly, we may also receive information about you from other sources, including third parties, business partners, our affiliates, or publicly-available sources.
Protected Health Information
When you set up an account with Caredocnow, you are creating a direct customer relationship with Caredocnow that enables you to access and/or utilize the various functions of the
Platform and the Service as a user. As part of that relationship, you provide information to Caredocnow, including but not limited to, your name, email address, shipping address, phone number and certain transactional information,
that we do not consider to be “protected health information” or “medical information”. However, in using certain components of the Service, you may also provide certain health or medical information that may be protected under
applicable laws. Caredocnow is not a “covered entity” under the Health Insurance Portability and Accountability Act of 1996, Public Law 104-191, and its related regulations and amendments from time to time (collectively, ” HIPAA”).
One or more of the Labs, Pharmacies or Medical Groups (as defined in our Terms and Conditions) may or may not be a “covered entity” or “business associate” under HIPAA, and Caredocnow may in some cases be a “business associate” of a
Pharmacy or Medical Group. It is important to note that HIPAA does not necessarily apply to an entity or person simply because there is health information involved, and HIPAA may not apply to your transactions or communications with
Caredocnow, the Medical Groups, the Providers, the Labs, or the Pharmacies. To the extent Caredocnow is deemed a “business associate” however, and solely in its role as a business associate, Caredocnow, may be subject to
certain provisions of HIPAA with respect to “protected health information,” as defined under HIPAA, that you provide to Caredocnow, the Medical Group or the Providers (” PHI”). In addition, any medical or health information that you
provide that is subject to specific protections under applicable state laws (collectively, with PHI, “Protected Information” ), will be used and disclosed only in accordance with such applicable laws. However, any information that does
not constitute Protected Information under applicable laws may be used or disclosed in any manner permitted under this Privacy Policy. Protected Information does not include information that has been de-identified in accordance with
applicable laws. By accessing or using any part of the Service, you are agreeing that even if HIPAA does apply to Caredocnow, the Medical Groups, the Providers, the Labs or the Pharmacies, any information that you submit to Caredocnow
Health that is not intended and used solely for the provision of diagnosis and treatment by the Medical Group and Providers, laboratory services by the Labs or prescription fulfillment by the Pharmacies, is not considered Protected
Information, and will only be subject to our Privacy Policy and any applicable state laws that govern the privacy and security of such information. For purposes of clarity, information you provide to Caredocnow in order to register
and set up an account on the Platform, including name, username, email address, shipping address and phone number, are not considered Protected Information.
Collection of Information
We collect any information you provide when you use the Service, including, but not limited to:
- Personally identifying information such as your name and contact data such as your e-mail address, phone number, and billing and physical addresses
- Your login and password and other account (” Account”) registration details
- Demographic data (such as your gender, date of birth and zip code)

- Photographic or video images submitted for identification or non-diagnosis or treatment purposes, including photographs of your driver’s license or passport
- Information about third parties that you refer to us (e.g., name, email, and/or other contact information, relationship)

If you use your mobile device to visit, access or use the Service, then additional categories of information that we collect may include
- Your name associated with your mobile device
- Your telephone number associated with your mobile device
- Your geolocation
- Your mobile device ID information
- With your express consent, your contacts and/or contact information (e.g., names, telephone numbers, physical addresses, email addresses, photos) stored on your mobile device
- With your express consent, information about third-party software applications on your mobile device (including, without limitation, general software apps, downloadable software apps, social media apps)
We also collect certain medical information on behalf of the Medical Groups and your Providers, which may include, but is not limited to:
- Health and medical data you submit for diagnosis or treatment purposes, including information in any questionnaires or surveys you complete for these purposes
- Previous doctors or other healthcare providers you visited
- Date of visit
- Images or videos you share for diagnosis or treatment purposes
- Communications with Providers
We may also receive information about you from our partners. For example, as part of our identity verification process, our vendor may send us information they have independently collected, such as your name, age, and estimated
location. Our marketing partners may also send us information about you, even if you have not visited or registered on our site. Caredocnow does not collect or create biometric information about you. To use some of our services,
however, we may be required to verify your identity. If you are asked to submit proof of identity (such as a driver’s license or passport) we may share that and the selfie you shared with us with our identity verification partner, who
may create biometric information about your face in order to verify that your selfie matches your proof of identity. Biometric information is not shared with Caredocnow and is deleted by our identity verification partner after
completing the identity verification. Caredocnow may receive information extracted from your photos, such as information from your driver’s license and the confidence that there is a “match” between your two photos. We use this
information to help verify your identity.
Cookies and similar technologies
A “cookie” is a small file created by a web server that can be stored on a user’s device for use either during a particular browsing session (a “session” cookie) or a future browsing
session (a “persistent” cookie). This file may include information about your internet protocol (IP) addresses, browser type, internet service provider (ISP), URLs of any referring or exited webpages, operating system, and/or date and
time stamps. We may use cookies to enable certain website functions, develop web browsing analytics, store and track your preferences, and enable the delivery of advertising and marketing, when applicable. For example, we may track the
number of “hits” or visits to the Site, the most popular times of day and sections visited, and the range of devices and browsers most commonly used to view the Site.
FIRST-PARTY COOKIES
Your placement of an order does not necessarily ensure that your order will be accepted. We may require additional information regarding your order if you have not provided all the information required to be completed. Once a properly
completed order and authorization of your form of payment is received, we will process your order and arrange for shipment. If, for some reason, it is determined that your Product(s) cannot be shipped within thirty (30) days following
receipt of a properly completed order, we will cancel your order and advise you of such action. We do not accept orders from dealers, wholesalers or other customers who intend to resell items offered on our Site.

WHAT SIMILAR TECHNOLOGIES ARE IN USE AND WHY DO WE USE THEM?

How long do we store your personal information?

How do we protect your information?

tracking of individual users’ browsing activities. We adhere to the standards set out in this Policy and do not currently respond to “Do Not Track” signals on the Site or on third-party websites or online
services where we may collect information.
